A YOUNGSTER almost fell from a moving train on to the tracks below because a toilet floor was missing.
The kid's mum managed to grab hold of him and pull him from danger after he headed to the loo on a train in Devon.
An investigation has revealed the floor has been removed while repairs were made to the vehicle's brakes and never replaced, leaving a potential death trap.
The train had been travelling at 20mph when the youngster opened the door and saw the wheels below on June 22, BBC reports.
The child suffered minor bruising, with both him and his mum left in shock.
South Devon Railway, which runs the Totnes Riverside to Buckfastleigh service, has said it is taking the investigation "extremely seriously".
The probe will look at safety measures and systems, with a full report to be published in due course.
The Office of Road and Rail has also launched an investigation.

South Devon Railway apologised in a statement and said: "On the day in question, something clearly went wrong with our safety control and hazard monitoring systems as evidenced by the incident having taken place – it simply should not have happened."
